Who We Are
Your Medical Home Community Care Clinic (YMHCCC) is a nonprofit medical clinic rooted in Bridge City, Texas, and committed to one belief: quality healthcare is a right, not a privilege.
We provide in-person and virtual primary care, preventive services, acute care, and wellness programs to individuals and families across Southeast Texas. Our clinic is funded through a unique combination of employer memberships, individual memberships, community donations, and local partnerships, allowing us to keep care affordable and accessible for everyone we serve.

* Additional Fees may apply for services that require a more complex/higher acuity of care and/or services not included with membership. All additional fees will be the responsibility of the member, not the employer.
* Membership is not available to beneficiaries of Medicare, Medicaid, CHIP, TriCare, or any other state or federal government healthcare program.
* All membership services performed will be at the provider's discretion. Pain Management Not Included. Limited Prescriptive Authority. MedSpa, Occ-Med, and Concierge Wellness Services are Not Included.
* In the event of a Pandemic or Natural Disaster, services and pricing are subject to change.
