About Us
Who We Are
Your Medical Home Community Care Clinic (YMHCCC) is a non-profit medical clinic located in Bridge City. We are committed to providing in-person and virtual, high-quality healthcare services to individuals and families in need. Our mission is to ensure everyone, regardless of income and insurance status, receives compassionate and comprehensive medical care.
At YMHCCC, we offer primary care, preventative health services, chronic disease management, and wellness programs tailored to our community’s unique needs. With a dedicated team of healthcare professionals and support from local partnerships, YMH serves as a medical home for underserved residents, improving health outcomes and fostering a healthier, stronger community.

* Monthly membership account must be set up on Auto-Pay.
* Additional Fees may apply for services that require a more complex/higher acuity of care and/or services not included with membership. All additional fees will be the responsibility of the member, not the employer.
* Membership is not available to beneficiaries of Medicare, Medicaid, CHIP, TriCare, or any other state or federal government healthcare program.
* All membership services performed will be at the provider's discretion. Pain Management Not Included. Limited Prescriptive Authority. MedSpa, Occ-Med, and Concierge Wellness Services are Not Included.
* In the event of a Pandemic or Natural Disaster, services and pricing are subject to change.
